WHAT IS SCIATICA?
Have you ever felt a numb tingling or hot burning sensation down your leg? It can be MISERABLE! You see, all the nerves that travel down your leg leave your spinal cord right in your lower back. Any occlusion or impedance on these nerves can result in these sensations, also known as sciatica.

WHAT CAUSES SCIATICA?
Anything that pinches on the nerve or affects its conductance can result in sciatica sensations. The most common causes I encounter in my clinic are disc bulges and stenosis (a narrowing of the vertebral canals).

It’s crucial to note that since not all sciatic nerve pains are caused by the same thing they CANNOT all be treated the same way! In fact what may be great for one type of pain may be awful for another.

For the purposes of this video, I’ll be demonstrating exercises to alleviate sciatica caused by the most-common problem that I encounter – a bulged or herniated lumbar spine disc.

THE BEST EXERCISES FOR SCIATIC NERVE PAIN
Treatment of bulged disc sciatic nerve symptoms is centered around alleviating pressure from the bulge on the nerve. This is best done with “directional preference” activities – in this case extension (or bending backwards). This is the treatment presented in this video.

CENTRALIZATION VS PERIPHERALIZATION
One more key principle to understand – any movement or exercise that causes pain to creep up the leg and come closer to the lower back (centralize) is an indication that you’re making good progress and those activities should be encouraged. Conversely – anything that causes pain to radiate further down your leg or intensify (peripheralization) should be discontinued immediately.

But there will come a time when your progress will plateau and stall out. Once we centralize the pain we must perform some core exercises to stabilize our spine to keep the pain away.

My pain is down my left leg. It does travel all the way down to my foot and toes but most of the time it stays in my left calf muscle. If I move my foot wrong it will cause a left crap and muscle spasms in my calf. It's very painful. I have ddd and had a discectomy 8 months ago. While doing on the of stretches today it cause a crap. Will these excercises help with the craps also?

My MRI of the lumbar spine demonstrates multilevel degenerative changes. I am a Probation Officer and I got injured in 2019 during a physical restraint which we all ended up on the ground (concrete) and suffered a lower back sprain/Strain … I received physical therapy for 3 weeks which was helping but my job denied my claim and I had to go back to the type of work without physical therapy… I did a QME that stated my injury was 100% work related but I didn't get the approval letter til Dec 2020. I had to transfer to a desk position in Aug 2020 and I gained 60lbs… I tried to restart the same physical therapy but it caused more pain. I was able to manage my lower back pain with over the counter meds when I was still doing the physical part of my job and but when I transferred to a desk position I started have extreme back pain that eventually led to a referral to a specialist. The special ordered an MRI which stated: MRI of the lumbar spine demonstrates multilevel degenerative changes.

At l4-l5 there is a small annular disc bulge however significant fluid within the facets and facet arthropathy contributes to bilateral neural foraminal narrowing.

L5-S1 disc osteophyte complex as well as facet arthropathy results in moderate left and mild right neural foraminal narrowing.

I was taken off work when I started having extreme pain in my right buttock, and I right hip, right thigh and tingling in my right toes. I was unable to walk without crutches and pain meds did not help.

I am so discouraged because I am once again being denied by my job saying the pain going down my right side is not due to my lower back injury 2019…

I'm a bit confused with my sciatica. I've been having pain down my left leg that stops at about the top of my knee but typically hurts most in the upper buttocks when I stand up from sitting position or bend over (I avoid this now). I recently found out about the pressup exercises and found relief after about 2-3 days of doing the exercise. Then, around the 5th or 6th day of continued exercises it started getting worse again. Now, about a week later the exercises are not providing the relief they did before (very demoralizing). I've now been experimenting with different positions/offsets with no avail. Not sure what to do now.. Do I just need to wait it out? Should I continue to do the exercises? I can't afford to find a doctor unfortunately..
